Pilot and ATC communications: How to.

Hi fellow FG'ers,

I was curious about the proper and most accepted ways of basic/essential communication between a pilot and an ATC are. Specifically, What to say to the ATC durning flight planning, filing a flight plan, Taxiing, and approach. Not sure if you necessarily have to report all that info, but would like to know how the 'pro's' do it. 8) I think I have an idea of how it's done, but I'd like some opinions as I am not a 'real' pilot and just an enthusiast interested in how it's really done.

If someone could list a basic mockup of examples of proper pilot and ATC communication for pre-flight and post flight that would be great. :D Or, if there is a link to a website that explains the do's and don'ts please post it!
