The purpose of this was to transport an engine when another 747 was off-base with a failed engine.

Sorry for the bad pun, but transporting them like that must be quite a drag. :P ;)

Anyone here know the design hours needed, the stress calcs required for such a thing....

It'd be quicker to send the engine by rickshaw.

If you watch 'top gear' you'll see a 747 on their runway used for 'a star in a reasonably priced car'. It's RR's flying test bed and can be regularly seen with 5 engines on it.

The calculations for loading, drag, performance etc. are the normal ones for carrying external (usually military) stores.

I believe the 747 was designed with ferrying an engine in mind. So this was not a new design designed specifically for this flight.
